Nazorg Optimalisatie Voor revalidanten met niet-Aangeboren Hersenletsel (NOVAH) - Aftercare optimalisation for people undergoing rehabilitation with acquired brain injury

In the NOVAH project, the department Roessingh Research & Development is developing a new aftercare programme for Roessingh, Centre for Rehabilitation, for people with acquired brain injury (ABI).

In collaboration with the University of Twente, the project is implementing recommendations from an earlier pilot. The core objectives of the NOVAH project are: a more central coordinating role for the aftercare nurse, promoting digital support to set up hybrid aftercare, and implementing principles for “joint decision-making” to better involve people undergoing rehabilitation and their loved ones in shaping the aftercare process. In addition to healthcare professionals, people undergoing rehabilitation and their loved ones are involved in the project through co-creation sessions, with the aim of developing a structured and personalised aftercare programme.

 

Tasks Roessingh

  • Setting up and conducting co-creation sessions with healthcare providers, people undergoing rehabilitation and their loved ones
  • Exploring opportunities for digital support: including video calling, updated online questionnaires, etc.
  • Developing practical tools to shape "shared decision-making"
