An internet-based telemedicine service for self-management of physical fitness of chronic patients

Roessingh Research and Development developed an innovative telemedicine service for the rehabilitation of chronic patients "physical reconditioning at home, remotely guided”. This is a web-based exercise program that allows patients to train in their home, but also remotely supervised by a therapist. The purpose of this telemedicine service is to support patients with their physical reconditioning, for both specific functions, such as the arm or hand, and the entire physical condition, and to coach the patient to restore and maintain their physical function. This training is integrated in the daily life of the patient, resulting in optimal integration and participation. For end-users, being the patient and the therapist / institution, it is expected that implementation of the service leads to better individual patient-tailored care, which enables patients to better manage their disease and which increases their quality of life.
The goal of the project Telerevalidatie.nl is implementing this new service in three rehabilitation centers in five different patient groups. The participating rehabilitation centers are: Merem, Reade and Groot Klimmendaal. They develop treatment protocols and deploy the innovative service in five diagnostic groups across the rehabilitation sectors pain, adults and children. At the end of this project there will be generic implementation knowledge available for broader implementation of the service widely in rehabilitation clinics in the Netherlands.

Project partners are: Merem, Reade, Groot Klimmendaal, Roessingh Telezorg Centrum, Roessingh Research and Development.
