ConditieCoach (CoCo)
A technology-supported health care service for self-management of physical fitness of elderly and chronic ill.
The innovation – ConditieCoach (CoCo) - is a technology-supported health care service for self-management of physical fitness. Elderly and chronically ill are thereby remotely supervised by health care professionals. CoCo supports rehabilitation and improvement as well as maintaining physical fitness. The communication between care users and health care professionals can fit into the rhythm of daily life with the help of CoCo. It thus reduces the burden of disease, it improves the self-management of the patient, provides the opportunity to live independently and (continue to) participate in employment, education or other activities in society. Especially the elderly and chronically ill who are supported for a longer time will benefit most. In addition, since support and information facilities are better organized, it can relieve care.

CoCo consists of:
1. Activity monitoring by use of a Personal Digital Assistant (PDA) and movement sensor
The movement sensor records the activity pattern of the patient and displays this real-time on the PDA at any time and place. The patient gets insight in his own activity pattern by looking at the PDA (continuously visual feedback). The activity data are sent to a database 'behind' a secure website via a wireless connection and are thus available for the caregiver.
2. Online individual exercise therapy
The patient has his own home exercise program exercises which are shown to the patient after logging in to the CoCo website. The exercises are illustrated by video and voice instruction. This web-based exercise program is specific to the patient (database of 200 videos). Besides this training function, the self-management program has other functions like communication and information facilities.
3. Telemonitoring and feedback
The caregiver can follow the performance of the patient: how long is the
patient physically active at home, how long does the patient need to perform his
exercise and is it correctly performed? Online support and feedback on the results of the patient is possible and exercises can be adjusted to the patient if necessary.
